FAP Forms

Below you will find the forms to make a submission to the Federal Appeals Panel.

  • If you would like to file a case with the FAP, please complete Form 1A or Form 1B.
    • Form 1A is used to appeal decisions made within the Federal Complaint Process.
    • Form 1B is for all other purposes.
  • If you are responding to a case that has been accepted by the FAP, please complete Form 2.
  • If you would like to file a procedural application, please complete Form 3.
    • Below are some examples of reasons for procedural applications to be submitted:
      • to request special arrangements for providing evidence,
      • to request permission to withold sensitive information from other parties,
      • to request an extenson of a deadline,
      • to request a change in directions already given.
  • If you are responding to a procedural application, please complete Form 4.
